LNC Wellness Center is committed to transparency, client rights, and ethical care. In accordance with Texas law, the following information is provided for current and former clients.

Requesting Your Health Records

Clients may request copies of their health records maintained by LNC Wellness Center.

How to request records:

  • Requests must include the client’s full name, date of birth, dates of service, and preferred method of delivery.

Record requests are processed in accordance with applicable state and federal laws, including HIPAA timelines.

Filing a Consumer Complaint

Clients have the right to file a complaint regarding access to health records or privacy concerns.
